I am announcing my departure from Baylor Scott and White Comprehensive Wound Care Center at Las Colinas effective as of October 17th, 2025 at 12pm. I am relocating to a different practice in Salt Lake City, Utah. It has been a pleasure providing for your health care needs.

 As of October 17th, 2025 at 12pm, Baylor Scott and White System will be responsible for all of my patients. However, you may seek medical care from another doctor if you so choose. If you decide to seek medical care from another doctor, please advise the practice as soon as possible.

 Your medical records are confidential, and a copy can be transferred to another doctor or released to you or another person you designate only through your permission. If you choose to see a different doctor not affiliated with Baylor Scott and White, please contact Heather Wheaton, practice  manager  at Baylor Scott and White Comprehensive Wound Care Center at Las Colinas, at  972-990-5222 as soon as possible so your medical records can be transferred to your chosen doctor. Until then, your medical records will remain on file at Baylor Scott and White System.
